It also states that they will offer amnesty to anyone who withdraws from the plan prior to the audit.
I can understand perhaps asking this info for new enrollees. Considering that several times a year we are required to send in proof the kids are still in school or a statement that someone else does not have a paying job- I think this is overkill on their part. Note they are not asking for proof of college enrollment according to this letter.
Is it just us or is something new blowing around minds of corporate America?

Interesting. Is this something new? Is this required by state/federal law? Marriage certificate? I guess they are looking for people not really married. Birth certificate? Trying to eliminate illegal aliena? How is it they can ask for a copy of your income tax form?
Send copies in the next few weeks? It could take longer than that to gather the information. For the first 10 or so years of our marriage we had no copy of the marriage certificate. Does everyone keep track of their birth certificates these days?
The insurance racketeers want to see your papers
Blue Cross and the University of Tennessee changed their health insurance offerings in January of this year, and my wife and I had to submit these same documents to prove that we were married. We did so really fast since she's been diagnosed with cancer, which means that, should our insurance lapse, she'd be uninsurable until she became eligible for the hated socialist Medicare program.
Now that I'm planning retirement later this year, we have to show the same documents all over again to get health insurance for both of us after I retire.
Get used to it, folks. The insurance rackets own both political parties; their whims and desires become law.

But you can get a copy of your Tennessee birth certificate from Vital Records, if you need it. I'm not sure what it costs now. The last time I got a copy, it was $7, but that was in the early 90's.

I've got a copy of my birth certificate, but Jim doesn't have a copy of his. He'd have to write to the state of Washington to get one. Also, I have no idea where our marriage certificate is.

further details unfolding. Tax return only for domestic partnership to show proof of dependency status. Kids will be required to send copy of birth certificates and marriage record proof required for spouse. There is a long list of acceptable items for children who were adopted or foreign born.
Thankfully, the documents go to the employer rather than the Plan Administrator. Considering the PA lost one claim of ours five times in six months really gave me concern about sending them any personal documents.

I have a copy of my birth
I have a copy of my birth certificate. I can't remember why I got it, but I was still living in Arkansas. I think it was $12, but I'm not going to swear to that.
I too keep warranty info and buttons. Can't say I do the personal worth thing, Tamara. But I can give you a rough estimate if needed.
If it meant I could have health insurance, I'd probably give 'em any of this stuff they wanted. I provide Interfaith with a copy of my income tax return every year so that I can show that I still qualify under their guidelines. And I have to attach income tax info to most of the patient assistance forms that I fill out for prescriptions. Of course, without insurance, a couple of my scripts would easily be close to $200 a month each.
